The information you provide will be treated as confidential and will only be accessible to your team's designated health professional and authorised Victorian State Team Officials, strictly on a need-to-know basis and where relevant to their responsibilities for your health, safety and wellbeing.

Your information will be used to assist with injury management, training modifications, recovery and any other appropriate support required during your preparation and participation in the National Championships.
